Focused Shockwave Therapy for Fibromyalgia & Myofascial Pain
Clinical Evidence & Research on Shockwave Therapy
Understanding Fibromyalgia & Myofascial Pain
Fibromyalgia and myofascial pain syndrome (MPS) are chronic pain conditions characterized by widespread muscle pain, tender points, and functional limitations. Myofascial pain involves trigger points with referred pain patterns, often affecting the neck, shoulders, and back. Fibromyalgia involves more widespread symptoms and often accompanies other conditions. Traditional treatments have limited efficacy, and patients seek alternative approaches. One randomised, sham-controlled trial studied focused shockwave for myofascial pain of the upper trapezius. It did not study fibromyalgia.
Focused shockwave therapy, also known as extracorporeal shock wave therapy (ESWT), has emerged as a promising non-invasive treatment approach for fibromyalgia & myofascial pain. This therapy uses high-energy acoustic waves to stimulate natural healing processes within the body.
About Focused Shockwave Therapy
Focused shockwave therapy, commonly referred to as ESWT, is a non-invasive therapeutic technique that delivers high-energy mechanical waves to targeted tissue areas. The therapy has a decades-long history in clinical applications and has been extensively researched across multiple medical disciplines.
Pulse Wave™ is the name of one focused shockwave device. It is not another name for the therapy, and none of the studies on this page used it.
Key characteristics of focused shockwave therapy include:
- Non-invasive delivery of mechanical energy to tissue
- Stimulation of natural biological healing processes
- Reduction of pain and inflammation
- Promotion of tissue regeneration and repair
- Well-established safety profile in clinical applications

PMID: 40609961
DOI: 10.1016/j.ctim.2025.103203
This trial studied myofascial pain syndrome of the upper trapezius. It did not study fibromyalgia. It used focused shockwave from another manufacturer, not Pulse Wave™.
Study Design & Methodology
Study Type: Randomized, sham-controlled trial of focused shockwave (fESWT)
Participants: 64 office workers (mean age 31.4) with myofascial pain syndrome of the upper trapezius
Treatment: Focused shockwave or sham focused shockwave, once a week for 4 weeks, at 4 Hz
Follow-up: Baseline, immediately after treatment, 2 weeks and 4 weeks
Outcome Measurements
Tissue stiffness (shear modulus by ultrasound elastography), pain (visual analog scale) and the Neck Disability Index.
Key Findings
Pain scores fell at every time point after focused shockwave (p<0.05)
Trigger-point stiffness dropped right after treatment (6.1 kPa, p=0.009)
Deeper tissue stiffness dropped by 4 weeks (5.3 kPa, p=0.004)
Summary of Results
| Outcome | Focused shockwave group | Sham group |
|---|---|---|
| Pain (VAS) | Decreased at all time points (p<0.05) | Decreased in the final two weeks |
| Trigger-point stiffness | −6.1 kPa immediately after (p=0.009) | Not reported as significant |
| Lower aponeurosis stiffness | −5.3 kPa at 4 weeks (p=0.004) | Not reported as significant |
| Neck Disability Index | Decreased after 4 sessions | Decreased; no difference between groups |
Limitations: The sham group also improved on pain and disability, so part of the effect may not be specific to shockwave. The authors call for more research on dose and number of sessions.
How Focused Shockwave Therapy Works for Fibromyalgia & Myofascial Pain
Focused shockwave therapy addresses myofascial pain through multiple mechanisms: directly treating myofascial trigger points by disrupting tense, contracted muscle fibers, promoting angiogenesis to improve muscle blood flow, reducing muscle tension and spasm through mechanical and neurological effects, and modulating pain signaling pathways. The stimulation of neural pathways may also contribute to pain reduction.
The therapeutic effect of focused shockwave therapy is thought to involve multiple biological pathways working synergistically. Understanding these mechanisms provides insight into why ESWT can be effective for various conditions including fibromyalgia & myofascial pain.
Biological Effects:
- Neovascularization: Creation of new blood vessel networks to improve tissue perfusion
- Angiogenesis: Growth of new blood vessels for enhanced nutrient delivery
- Mechanotransduction: Cellular response to mechanical stimulation
- Anti-inflammatory signaling: Reduction of inflammatory mediators
Frequently Asked Questions
Is Pulse Wave the same thing as shockwave therapy?
No. Shockwave therapy is the treatment. Pulse Wave™ is one focused shockwave device. The research on this page studied shockwave therapy with other devices. None of it used Pulse Wave.
How long does focused shockwave therapy treatment take?
Treatment duration varies depending on the specific condition and treatment protocol. Typical sessions last 15-30 minutes, with most protocols involving multiple sessions over several weeks. The exact number of sessions and frequency will depend on your specific condition, the severity of symptoms, and individual response to therapy. Your healthcare provider will determine the appropriate treatment schedule.
Is focused shockwave therapy painful?
Focused shockwave therapy is generally well-tolerated. During treatment, you may experience mild to moderate sensations at the treatment site. Most patients find the procedure tolerable without requiring anesthesia or significant pain relief. Any temporary discomfort typically resolves quickly after the session.
How soon can I expect results from shockwave therapy?
Timeline for results varies among individuals. Some patients report symptom improvement within days to weeks of starting treatment, while for others, maximal benefits develop over several weeks to months. Consistent treatment following the prescribed protocol optimizes outcomes. Clinical research suggests peak efficacy often occurs within 4-12 weeks post-treatment initiation.
What are the side effects of focused shockwave therapy?
Focused shockwave therapy has an excellent safety profile with minimal adverse effects. Some patients may experience temporary mild redness, swelling, or discomfort at the treatment site, which typically resolves within hours to days. Serious side effects are rare when treatment is administered by qualified healthcare professionals using appropriate protocols and devices.
Additional Clinical Context
Fibromyalgia is a complex central sensitisation syndrome, and the trial on this page did not include fibromyalgia patients. It studied trigger points in the upper trapezius of office workers with myofascial pain.
This clinical evidence supports focused shockwave therapy as a valuable non-invasive option for managing fibromyalgia & myofascial pain. The research demonstrates measurable improvements in both pain and functional outcomes, making shockwave therapy a significant addition to the treatment landscape for this condition.
